Resumen: Objective: This study aimed to provide a comprehensive overview of self-medication practices among students by conducting a bibliometric analysis of the available scientific literature. This research highlights the importance of promoting safe and responsible healthcare behaviors among students. Methods: A systematic search was conducted in the Scopus database to retrieve all peer-reviewed English articles and reviews published from 1968 onwards. The retrieved documents were analyzed to identify publication trends, citation counts, top journals, geographical distribution, and emerging research themes. Results: The findings indicate a significant increase in published literature about student self-medication over the past fifteen years. However, it was observed that the citation count for these documents was lower than expected, suggesting a need for increased attention toward this critical topic. The analysis also identified several hot topics in student self-medication, including the misuse of over-the-counter medications, dietary supplements, and psychoactive substances. The inappropriate use of antibiotics and the self-medication of mental health issues, such as anxiety and depression, were also identified as significant problems. Conclusions and recommendations: Self-medication among students is a complex and critical issue that requires immediate attention. This study highlights the urgent need for greater awareness and education regarding responsible self-medication practices among students. New policies, interventions, and strategies should be developed to address malpractices, misconceptions, and harmful practices related to self-medication. Educational institutions and health authorities should play a crucial role in providing students with mental health resources and support services... (AU)

Título: Evaluation of the Association between Inhaler technique and Adherence in Asthma Control: Cross-Sectional Comparative Analysis Study between Amman and Baghdad

Resumen: Purpose: Asthma is a chronic condition affecting millions of people all around the world. Asthma has no cure, but disease control is essential and highly recommended. However, the available tools for asthma control assessment don't include factors such as inhaler technique and adherence. This study aimed to assess the correlation between inhaler techniques, adherence, and level of asthma control in two different healthcare settings; Jordan and Iraq. Patients and methods: A cross-sectional observational study was con-ducted over six months, from January to August 2018, in two public hospitals in Amman (Jordan) and Baghdad (Iraq). Asthmatic patients were interviewed to assess their inhaler technique, adherence, and asthma control. The researcher personally visited both public hospitals, conducting face-to-face interviews with patients at the hospital outpatient clinics. Validated questionnaires were used for patient assessment, including demographics, asthma history and medication use, the patient's inhaler technique, adherence, and asthma control. Results: A total of 300 patients entered the study, with a mean age of 45.54 ± 13.71. The asthma control test showed very poor asthma control for patients living in both countries (Amman n=78 (52.0%) vs. Baghdad n=106 (70.0%)). An asthma knowledge assessment showed that most asthmatic patients in both countries didn't follow their asthma medication plan (Amman n=78 (52.0%) vs. Baghdad n=93 (62.0%). Conclusion: In both Jordan and Iraq, asthma patients were found to be poorly controlled. Knowledge of patients was inadequate, probably leading to the poorly managed chronic disease. The results of this study highlight the significance of the pharmacist's role in recognizing asthmatic patients requiring assistance. Furthermore, they underscore the pharmacist's pivotal contribution to delivering patient education and counseling, ultimately resulting in enhanced asthma control. (AU)

Resumen: Purpose: The complete awareness of policymakers about air pollution can play a significant role in dealing with this environmental threat. This study aimed to examine the awareness and perception among the policymakers regarding air pollution in Jordan. Methods: This is a cross-sectional study conducted in Jordan. The participants included in this study were governmental and non-governmental officials from different sectors. An interview and questionnaire were used to examine the extent of knowledge of these decision-makers regarding air pollution types, sources, and threats. Results: Most participants acknowledged that air pollution in Jordan was a huge problem. Most participants (90%) were familiar with the term air pollution, and regularly read about different contaminants that cause air pollution. However, most of the participants had a low level of knowledge about air contaminant types in Jordan. The least known pollutants were PM2.5 (Particulate Matter 2.5 microns in width) and PM10 (Particulate Matter 10 microns in width). Only 7.5% of the participants knew about PM2.5, while 12.5% knew about PM10. Conclusion: Policymakers have the full authority to initiate and implement policies that intend to limit air pollution. Their complete awareness of air pollution can result in conducting certain protocols to approve new policies and legislations that can improve the air quality in Jordan. Unfortunately, no previous studies were conducted to analyze policymakers' knowledge regarding air pollution in Jordan, and this study's results hoped to reflect the importance of air pollutants and their greater significance on public health. Título: Inappropriate Medications Use among Elderly Cancer Patients According to Beer's Criteria

Resumen: Background: The global population of elderly has substantially increased in recent years due to heightened life expectancy and improved survival rates for numerous diseases, including cancer. Cancer treatment often entails complex regimens involving multiple medications. Additionally, advancing age is associated with a higher prevalence of co-morbidities, rendering older individuals more susceptible to inappropriate medications use and adverse drug events. Objective: We aimed in our study to examine the extent of Potentially Inappropriate Medications (PIMs) prescribing and factors associated with more PIMs instances in elderly cancer patients. Methods: The data of this study was evaluated utilizing medical records of included study subjects and was conducted over more than 3 years period (January 1, 2019 to January 31, 2022) at King Abdullah University Hospital, Al Ramtha, Jordan. Beer's criteria 2019 was used to evaluate and identify the potentially inappropriate drugs prescribed and used among elderly cancer patients. Results: A total number of 250 geriatric cancer patients were included in this study. The mean age of the patients was 73.4 years. Males represented 50.4% of the total patients (n=126). The average number of medications reported was 10.7 medications. Eighty three percent (n=203) of patients had polypharmacy (prescribed at least five medications or more), A total of 179 medications were considered inappropriate according to the 2019 updated BEERS criteria and 71.6% of patients (n=179) received at least one PIM. The most common classes of PIMs were gastrointestinal medications (e.g., metoclopramide). Conclusion:According to this study, the incidence of PIMs in geriatric oncology practice is concerning, and extra consideration should be given to reduce any risks associated with this kind of prescribing in elderly cancer patients. Polypharmacy was found to be a major predictor of PIM prescription in this research. (AU)

Título: Comparative efficacy and safety of anti-PD-1 therapies used in metastatic colorectal cancer: a systematic review and network meta-analysis

Resumen: Objective: Systemic studies on anti-PD-1 therapy in patients with metastatic colorectal cancer (mCRC) with microsatellite instability or mismatch repair defects are lacking. We aimed to summarize the evidence regarding the efficacy and safety of pembrolizumab, nivolumab, ipilimumab, and tislelizumab in mCRC. Methods: Network meta-analyses (NMAs) can provide comparative efficacy and safety data for clinical decision-making. In this NMA, eligible publications from PubMed, EMBASE, Web of Science, and Cochrane Library from 2016 to April 2023 were identified through a systematic literature review. Literature screening and data extraction were performed according to established criteria. The quality of the literature was evaluated using the Cochrane risk of bias tool, and statistical analysis was performed using Revman5.4 and R language. The main outcome indicators, DCR, ORR, PFS, and OS, were used to evaluate the effectiveness of the drugs, and the main outcome indicators AE and SAE were used to evaluate the safety of each program. Results: Fifteen studies with a sample size of 798 patients were included. In terms of effectiveness, the disease control rate DCR of PD-1 inhibitors was 0.727[95% CI:0.654-0.794]; objective response rate ORR was 0.448[95% CI:0.382-0.514]; and the 1-year progression-free survival rate was 0.551[95% CI:0.458-0.642]. The 1-year overall survival rate was 0.790[95% CI:0.705-0.865]. The adverse events associated with anti-PD-1 were 0.567[95% CI:0.344-0.778] in terms of safety. The total incidence of grade 3 or higher adverse events was 0.241[95% CI:0.174-0.313]. In the subgroup analysis results, the incidence of DCR in the nivolumab + ipilimumab group was 0.826[95% CI:0.780-0.869], the ORR was 0.512[95% CI:0.377-0.647], and the PFS was 0.668[95% CI:0.516-0.804]. Título: A qualitative study on the process of dietitians working in pharmacies to practice dietary habit improvement support in their daily work

Resumen: Background: Recently in Japan, there have been some cases of pharmacies staffed with dietitians in an effort to improve their health support functions. However, there have been few reports on the effects of these efforts, and it has been pointed out that dietitians may not be fully exercising their professional abilities. We conducted a before-and-after comparison study in 2022 to confirm the effectiveness of collaboration between pharmacy dietitians and pharmacists in supporting patients with type 2 diabetes. Objective: This study aimed to investigate the process by which dietitians who participated in our research project became accustomed to providing continuous dietary counseling as a matter of habit. Methods: Semi-structured interviews were conducted with three dietitians and two pharmacists. The main questions asked were about 1) the nature of their work before they began providing the interventions, 2) the aspects they paid attention to while providing the interventions, 3) what they felt they had changed during the intervention period, and4) what they considered obstacles to providing nutritional guidance in their pharmacies. Results: The responses of the participants regarding the nature of their work before they began providing interventions were mostly related to their ôwork as a dispensing clerk.ö The dietitians also indicated their desire for more continuous dietary support. The responses of the participants regarding what they felt had changed during the intervention period included numerous references to having an ôopportunity for trial and errorö and obtaining the ôcooperation of staff.ö In the responses regarding what the participants considered obstacles, the dietitians mentioned ôrecognition by patientsö and ôan environment in which they can consult with patients.ö... Resumen: Background: Tamoxifen is a widely prescribed selective estrogen receptor modulator (SERM) used for the treatment and prevention of hormone receptor-positive breast cancer. While tamoxifen has proven effective in lowering the chance of cancer recurrence, current data point to a possible link between tamoxifen treatment and unfavorable psychological consequences. Objective: In this study, the World Health Organization (WHO) pharmacovigilance database will be analyzed to determine the frequency and kind of psychiatric disorders linked to the use of the tamoxifen as well as Kosovo pharmacists' awareness of these reported adverse effects. Methods: The WHO pharmacovigilance database, VigiBase, was queried for adverse drug reaction reports related to tamoxifen use between 1978 and 2022. A cross-sectional survey of 70 community pharmacies operating in Kosovo was conducted as part of the data collection between January 1 and March 1, 2023, with the goal of comparison. Results: The search of VigiBase yielded a total of 1746 reported cases of psychiatric disorders. The data were further classified based on the type of psychiatric adverse effect, including depressive disorders, anxiety disorders, cognitive impairment, and other psychiatric symptoms. Depressive disorders were the most frequently reported adverse event, accounting for 30% of the cases. Anxiety disorders and sleeps disorders were also prevalent, comprising 11% and 7% of the reported cases, respectively. Other psychiatric symptoms such as mood swings, insomnia, and confusion were observed in 32% of the cases. In Kosovo, 30% of pharmacist's linked tamoxifen with depression, 25% with anxiety, 56% with sleep disorders, 53% insomnia, and 18% confusion. Conclusions: Based on the WHO pharmacovigilance database and knowledge of Kosovo pharmacists for these documented adverse effects, this study analyzes tamoxifen-induced psychiatric disorders... (AU)

Resumen: Objective: To develop a comprehensive methodology for the optimal assessment of the share of the use of medicines, based on the procedure for ranking drugs according to the pharmacoeconomic point scale and the minimax criterion was applied. Methods: The author's approach is based on the minimax principle and allows solving the problem of optimizing the pharma drug portfolio based on available data, without the need to obtain the parameters of the Markowitz model associated with correlation analysis of data. Results: The authors obtained the optimal distribution of medicines in group A, B: 37% to 63%, which the authors consider a promising recommendation for a pharmaceutical company. The use of a similar approach, which does not contradict the Markowitz methodology, but allows us to reasonably accept the parameters of the model and give the optimal solution for the share distribution of drugs in medical practice. Conclusion: These mathematical tools, justified and equipped with an alternative confirmation, the minimax task can and should take a significant place in the complex pharma-analytical methodology of the management of large companies supplying concomitant drugs to the Russian and foreign market. Resumen: The prevalence of 3-M syndrome remains unclear owing to its rarity and the limited number of reported cases in the medical literature. To date, approximately 100 cases of the disorder have been documented in MedlinePlus Genetics. Here, we present the first case study report from Jordan of a boy diagnosed with 3-M syndrome at 9 months of age via karyotyping. The patient exhibited distinct facial features, severe prenatal and postnatal growth retardation, and normal mental development. As rare genetic autosomal recessive mutations are common where consanguineous marriages are prevalent, raising awareness of such rare genetic diseases is critical. This paper aims to provide a case report on 3-M syndrome and a literature review. (AU)

Resumen: Background: Indonesia has recently implemented a national policy to ensure equitable access to medicines, promote their rational use, and maintain a reliable and quality supply, specifically for essential medicines. Several organizations have conducted evaluations on essential medicines use but have yielded varying results and cannot reflect the actual situation. Objectives: This study aims to discover the current situation regarding essential medicines and identify the most important factors to be considered during future indicator-based evaluations in health facilities in Indonesia. Methods: This qualitative study was carried out using FGDs and interview from January to February 2022. The sample population consisted of ten experts selected based on predetermined criteria. The discussions were recorded and transcribed verbatim in the original language, thematically coded with Nvivo, and analyzed for common themes. Results: This study found 32 factors related to the use of essential medicines in Indonesia, divided into three categories of components, namely access, medicine handling quality, and rational use. Furthermore, a total of 10, 8, and 14 main factors were related to access, handling quality, and rational use, respectively. The discussion provided various perspectives on measuring drug use, specifically essential medicines. Based on expert opinions, evaluating the utilization of essential medicines by relying on existing guidelines was insufficient due to superficiality and irrelevance within the Indonesian health system. Conclusion: Based on the results, one of the crucial factors to consider during evaluation was the accessibility of medicines, which encompassed their availability in health facilities and affordability to patients...
