Fundamento: La aparición de angioedema hereditario debido a alteraciones genética que afectan a la síntesis de otros factores del complemento distintos del C inhibidor es un hecho infrecuente; aún es más rara su asociación con urticaðria. Tras detectar valore bajos de C2 y C en una mujer de 22 años que consultó por angioedema facial, eritrodermia y urticaria, se planteó el estudio familiar. Métodos: Se analizaron las fracciones del complemento en los miembros de la familia. Las fracciones e, inhibidor, e, y C del complemento en suero se determinaron por nefelometría. La fracción C2 e determinó por inmunodifusión radial. La actividad funcional del C, inhibidor (técnica de sustrato eromogénico) y CHso se determinaron por e pectrofotometría. Resultados: Se detectó una disminución de la fracción C2 en todos los miembros de Ja unidad familiar. En ambos progenitores la alteración era selectiva para dicha fracción. La paciente presentaba un déficit parcial de las fracciones C2 y Ce En las tres hermana se encontraron valores bajos de C2, e, y CJ. No se detectó alteración cuantitativa ni funcional del e, inhibidor ni de CHso en ninguno de los miembros de la familia. La determinación de C2 en el cónyuge de la paciente resultó normal. Conclusiones: El caso estudiado podría tratarse de una heterocizocia con expresión de un gen nulo o de un trastorno atípico de la regulación (AU)
Background: Familial hereditary angioedema due to genetic changes affecting the synthesis of complement components other Than e, inhibitor is an infrequent phenomenon; its association to urticaria is even less frequent. After the detection of low C2 and C levels in a 22ðyearðold woman con ulting because of facial angioedema, erythrodermia and urticaria, a familia! tudy was indicated. Methods: The complement fraction were analysed in alimembers of the family unit. The C1 inhibitor, e, and C. fractions were quantified by nephelometry, and the C. fraction by radial immunodiffusion. The functional C, inhibitor activity (chromogenic substrate technique) and the CHso were assessed by spectrophotometry. Results: Low C2 levels were detected in ali the member of the family unit. In both paren! , the change was selective for that fraction. The consulting patient (index case) evidenced a partial deficiency of the C2 and C. fractions, and her three sisters had low levels of Ci, C and C. No quantitative or functional derangement of the e, inhibitor or of CH50 was detected in any of the members of the family. The C2 quantitation in the consulting patient's pouse yielded normal results. Conclusions: The studied case might be one of heterozygosity with expresion of a null gene, or one of an atypical regulation di order (AU)
